Middleburg Rehab & Nursing is a great facility! They take great care of the residents, the nursing team is great & led by a great DON and ADON Sierra & Jada. The therapy team is amazing & has the best leader Janet! She cares so much about the residents. Luis is great too and so smart! I would tell anyone to come here! The food is amazing and Donna makes sure you feel as close to home as possible! It’s so clean & Georgette is always running and working hard to make sure everyone is happy & taken care of! Discharges are so smooth too! Byron & Bliss will make sure you have everything you need to go home! Love this facility so much!

I can’t say enough wonderful things about this facility! The staff are absolutely amazing and truly go above and beyond to make sure their patients feel cared for, respected, and comfortable. You can tell they genuinely care about the people they serve, and the level of compassion and professionalism is outstanding. The facility itself is beautiful, welcoming, and very well maintained. Another huge plus is the quality of the specialty physicians they are all knowledgeable, attentive, and wonderful to work with. It’s clear that this facility is committed to providing exceptional care and creating a positive experience for both patients and their families. Highly recommend!
I recently visited this nursing home and was genuinely impressed by how clean and well-maintained the entire facility is. From the moment I walked in, everything looked spotless and organized, with no unpleasant odors or clutter—something that really stands out in a care environment. The common areas were bright and welcoming, and residents appeared comfortable and well cared for. The staff were friendly and attentive, taking the time to greet visitors and interact kindly with residents. It was clear that cleanliness and hygiene are a top priority here, which gives a strong sense of safety and professionalism. Overall, the atmosphere felt calm, respectful, and thoughtfully managed. I would feel confident recommending this facility to others looking for quality care in a clean and supportive environment.
My mother was here for over two months & we had a great experience. The facility is extremely clean. My mom’s room was cleaned each & everyday! Joe & his staff do a great job. The Doctors, nurses, CNA’s, PT team, front desk staff, kitchen staff, activities team, housekeepers, social workers all went above & beyond to make sure my mom had a great experience while staying at this facility. The activities team would come to her room & do her nails. During a very hard time, they made us feel at home & treated my mom with care. Your facility was a blessing to our family. Thank you!
DO NOT GO unless you can be your own medical advocate. You must be VIGILANT!! I spent nearly 2 months in the Skilled Nursing Center during the summer of 2026 for IV antibiotics following surgery. The room was clean and housekeeping visited every day and were very friendly. The assistance call buttons in rooms down my hallway would be answered within 5 minutes, but regularly not answered for 20 or 30 minutes. More than once, I needed to confirm or correct the medications I was being given to ensure I was getting all the meds. prescribed and on more than one occasion, my nurse administered and tended to my IV antibiotic line without gloves. Patient bathing is "by request". If you don't ask, you don't get bathed. Nurses and CNAs don't seem to communicate with each other from shift-to-shift and I constantly had to reexplain my reason for being there. All they seem to know is in their computers. It was a 50 / 50 shot if the staff appeared happy to be there. Quite often, staff appeared depressed with their work situation or inconvenienced by the need to care for the patient. I got through, I got my medicine and I received only a small tear in my incision during some rough bed changing by a CNA, but being my own advocate was a large part of that success. If your loved one can't advocate for themselves in your absence, THINK TWICE. I would go hours without anyone even checking in on me and at least once, I had gone 24 hours without my catheter bag being monitored or emptied. If your loved one goes, they will get the care they need, but you MUST visit daily or even multiple times daily to monitor their care and condition.
